Appendicitis symptoms: where does the pain start?

Appendicitis needs urgent hospital treatment. Absence of the classic pain pattern does not rule it out.

Educational guide based on the sources below · 30 October 2026

Pain pattern and associated symptoms

Pain often begins around the belly button and moves to the lower right abdomen. Nausea, loss of appetite or fever may occur. Children, older adults and pregnant people may have different symptoms.

Diagnosis and action

Worsening or persistent pain needs prompt assessment. Examination, tests and sometimes ultrasound or CT help establish the diagnosis. Severe widespread pain, confusion or breathing difficulty require emergency care.

Frequently asked questions

Does appendicitis always cause fever?

No. Absence of fever does not rule it out.

Can I diagnose it by pressing my abdomen?

No. Home tests are not reliable and must not delay assessment.

Does all right-sided pain mean appendicitis?

No. Urinary, bowel and ovarian conditions or ectopic pregnancy are other possible causes.
